Calvert Family Practice Associates Announcement
Calvert Family Practice Associates is pleased to announce that as of June 29th, 2015, we became part of Calvert Physician Associates, the provider network of the Calvert Health System. Our new practice name is Calvert Primary Care - Prince Frederick.
This transition opens a new chapter for CFPA and marks the further development of Calvert Health System's commitment to a strong primary care base for residents of Calvert County.
Area families count on Calvert Memorial Hospital for outstanding medical care and now CFPA is an official part of the Calvert Health System family.
Patients of Calvert Family Practice Associates will see no change in our long time office staff or in access to their health care provider, who will continue to coordinate their medical care. The practice will welcome new providers in the coming months as we sadly say goodbye to Dr. Bill Duggan and to Heather Montgomery, CRNP. We will miss them greatly. The practice, which will remain at 110 Hospital Rd. Prince Frederick, will now be called "Calvert Primary Care - Prince Frederick".
Over the next weeks, we will share more information on what you can expect as we make this transition. Please be assured that we will make every effort to make it seamless for you.
Our highest priority is providing excellent care to our patients, and we expect to continue to do under our new name of Calvert Primary Care - Prince Frederick.

An Internet-based Patient Portal simplifies communication with your CPC provider and eliminates time-consuming phone calls to the practice. Whether you want to schedule an appointment or review your latest test results, the Patient Portal delivers the information you need through a convenient, easy-to-use Patient Portal.
This internet-based Patient Portal is a secure, confidential and easy-to-use website giving you 24-hour access to review health information sent from the CPC office. Secure messages and information can be viewed only by those individuals who enter the correct username and password to log-in to the Calvert Primary Care NextGen Patient Portal site. By signing up and enrolling with the NextGen Patient Portal, you will be able to:
- Correspond online with CPC providers
- Request appointments and receive appointment reminders
- Receive important health information that is sent from your physician's office, including medications, immunizations and test results.
- View medication lists and request prescription refills
- Obtain educational health information
- Maintain account information including user name, password, access privileges and email address.

Patients and their legal guardians can sign up for Patient Portal in accordance to the Terms and Conditions of the website. Just tell your provider who you would like to enroll and you are on your way to connecting with your provider.

You can sign up and register for the CPC NextGen Patient Portal by providing the office with your name and email address at check-in during your next visit or stop by our office. In turn, our staff will give you an instruction sheet that includes an activation code, known as an enrollment token. The enrollment token will allow you to log in to the system and create your own private user name and password.
