About our Medical Staff
At Calvert Family Practice Associates, our entire focus is on caring for you and your family in a friendly, welcoming, compassionate environment. To that end, we have built a medical practice in which good doctors and nurses have the freedom to practice good medicine; in which proven technologies help us deliver proven treatments; and in which patients feel cared for, respected and valued as individual human beings. The mission for our medical staff is to provide their patients with personalized care and service in a private practice setting.
We work hard to provide you and your family with an enhanced and modern view of what a family medicine practice should be. While we continue to provide care for illness and unexpected health problems, our focus continues to stress preventive health care, and putting the health needs and concerns of our patients first.
Our medical staff consists of Family Physicians, Board Certified by the American Board of Family Medicine and a Certified Family Medicine Nurse Practitioner. Family practice training requires three or more years in specialized residency training after medical school. To become certified by the American Board of Family Practice, a family practitioner is required to take a lengthy and detailed national examination. All Calvert Family Practice physicians are members of the American Academy of Family Physicians.

J. Michael Brooks, M.D. attended college at the University of Notre Dame and is a graduate of the Wayne University School of Medicine in Detroit, Michigan. He completed an internship and Family Practice Residency at the U.S. Naval Hospital in Charleston, SC. Dr. Brooks served in the United States Navy as a Family Physician and was last stationed at the Cherry Point Naval Air Station in Havelock, NC.
Dr. Brooks joined Calvert Family Practice Associates in 1990. Dr. Brooks holds Privileges at Calvert Memorial Hospital in Prince Frederick and has served as chairman of the hospital’s department of family medicine multiple times as well as numerous other committees over the past since 1990. He was a member of the CMH Board of Directors from 1996-2003. Additionally, he was recognized twice by Calvert Memorial Hospital for his leadership and service to the community. In 2004, he was honored for his work on the CMH Physician Leadership Development Program and again in 2005 for his participation in the athletic programs of Calvert County. In 2007, Dr. Brooks was recognized as “Volunteer of the Year” by Calvert County Public Schools for his work at Huntingtown High School.
He is a Fellow of the American Academy of Family Physicians and a member of the American College of Physician Executives.
In 2012, the Calvert Memorial Hospital board of directors named Dr. Brooks of Calvert Family Practice Associates as Vice President of Medical Affairs.
Dr. Brooks is Board Certified in Family Medicine with recertifications in 1994, 2000, 2006 and 2013.
Dr. Brooks and his family live in Saint Leonard. He enjoys hiking, biking, athletics and family time. He is a fan of the University of Notre Dame Fighting Irish. Dr. Brooks is a member of St. John Vianney church in Prince Frederick.

Kimberly D. Larsen, M.D is a graduate of Georgetown University and also the Georgetown University School of Medicine. She completed a Family Practice internship and residency at the Dwight D. Eisenhower Army Medical Center in Fort Gordon, GA. Dr. Larsen continued to serve as a Family Physician with the U.S. Army.
Dr. Larsen joined Calvert Family Practice Associates in 1997. Her special areas of medical interest are women and children's health and asthma management.
Dr. Larsen is a Board Certified Diplomate of the American Board of Family Medicine, was recertified in 2000 and 2007, and has been nominated as a Fellow of the American Academy of Family Practice. Dr. Larsen holds consulting privileges at Calvert Memorial Hospital served as past Chairman of the Department of Family Medicine.
Dr. Larsen and her husband reside in Huntingtown with her two children. She enjoys volunteering in her local school and church community, and time with her family.

Michelle A. Folsom, M.D. joined the CFPA medical practice in August, 2008. Dr. Folsom, is a graduate of the University of Maryland and also the University of Maryland School of Medicine. She is a 2008 graduate of the Thomas Jefferson University residency program in the Department of Family and Community Medicine.
Dr. Folsom grew up in Clinton, Maryland attending Bishop McNamara High School and then the University of Maryland, College Park. She majored in Physiology and Neurobiology as an undergraduate, then attended the University of Maryland, School of Medicine and graduated in 2005. She spent three years in Family Medicine Residency in Philadelphia at Thomas Jefferson University Hospital.
In her spare time, she enjoys baking and reading and numerous outdoor activities, including softball and tennis and jogging and cycling. Her interests within medicine include adolescent and women's health issues, as well as newborn care.
Dr. Folsom resides in Chesapeake Beanch and enjoys living and working in the Calvert County community.

Calvert Family Practice Associates is proud to announce that William M. Duggan, M.D. joins our medical practice September 2012.
Dr. Duggan grew up in Ireland and was pursuing a promising career in soccer before injuries led him to the medical profession. He attended The Royal College of Surgeons and graduated in 2005. He spent several years in surgical training before finishing his Family Medicine Residency with the Cleveland Clinic at Fairview Hospital in Ohio.
He has special interests in Sports medicine, sports nutrition and musculoskeletal health. He has provided sports coverage at both the high school and collegiate level in Cleveland Ohio. He is especially enthusiastic about promoting health and well being among adolescents and has a special interest in teen and adolescent health.
Dr Duggan and his family reside with in Annapolis and he remains an avid soccer player, middle distance runner and fitness enthusiast.
Dr. Duggan looks forward to working in the Calvert County community. Dr. Duggan joins Board Certified family physicians Dr. Michael Brooks, Dr. Kimberly Larsen, Dr. Michelle Folsom and Family Medicine Certified Nurse Practitioner Heather Montgomery in providing professional family care for patients of all ages. We are excited to welcome Dr. Duggan to the CFPA team.

Heather Montgomery, CRNP became a member of Calvert Family Practice Associates medical provider staff February, 2007. Heather works closely with all the CFPA Board Certified family physicians to provide professional family care for patients of all ages.
A Calvert County native, Heather Montgomery, CRNP, is a Cum Laude graduate of the Towson University College of Health Sciences. She achieved her Master of Nursing degree and graduated from Catholic University Nurse Practitioner Program in 2002.
She is a member of Maryland Coalition of Nurse Practitioners and The American Academy of Nurse Practitioners.
Heather's duties as a Family Nurse Practitioner include providing comprehensive primary health care to pediatric, adult and geriatric patients, including:
- Providing diagnosis and treatment for acute health problems.
- Comprehensive management of health care through complete physical exam, diagnostic testing, pharmacologic treatment, patient counseling and appropriate specialty referral.
