Our medical staff consists of Board Certified Family Physicians and Certified Family Medicine Nurse Practitioners. All Calvert Family Practice physicians are members of the American Academy of Family Physicians.

J. Michael Brooks, M.D. FAAFP

J. Michael Brooks, M.D. attended college at the University of Notre Dame and is a graduate of the Wayne University School of Medicine in Detroit, Michigan.  He completed an internship and Family Practice Residency at the U.S. Naval Hospital in Charleston, SC.  Dr. Brooks served in the United States Navy as a Family Physician and was last stationed at the Cherry Point Naval Air Station in Havelock, NC.

Dr. Brooks joined Calvert Family Practice Associates in 1990.  Dr. Brooks holds Privileges at Calvert Memorial Hospital in Prince Frederick and has served as chairman of the hospital’s department of family medicine multiple times as well as numerous other committees over the past since 1990. He was a member of the CMH Board of Directors from 1996-2003.  Additionally, he was recognized twice by Calvert Memorial Hospital for his leadership and service to the community. In 2004, he was honored for his work on the CMH Physician Leadership Development Program and again in 2005 for his participation in the athletic programs of Calvert County. In 2007, Dr. Brooks was recognized as “Volunteer of the Year” by Calvert County Public Schools for his work at Huntingtown High School.

He is a Fellow of the American Academy of Family Physicians and a member of the American College of Physician Executives.

In 2012, the Calvert Memorial Hospital board of directors named Dr. Brooks of Calvert Family Practice Associates as Vice President of Medical Affairs.

Dr. Brooks is Board Certified in Family Medicine with recertifications in 1994, 2000, 2006 and 2013.

Dr. Brooks and his family live in Saint Leonard.  He enjoys hiking, biking, athletics and family time.  He is a fan of the University of Notre Dame Fighting Irish.  Dr. Brooks is a member of St. John Vianney church in Prince Frederick.

Michelle Folsom Elder, M.D.

Michelle Folsom Elder, M.D. joined the medical practice in August, 2008. Dr. Folsom Elder, is a graduate of the University of Maryland and also the University of Maryland School of Medicine. She is a 2008 graduate of the Thomas Jefferson University residency program in the Department of Family and Community Medicine.

Dr. Folsom Elder grew up in Clinton, Maryland attending Bishop McNamara High School and then the University of Maryland, College Park. She majored in Physiology and Neurobiology as an undergraduate, then attended the University of Maryland, School of Medicine and graduated in 2005. She spent three years in Family Medicine Residency in Philadelphia at Thomas Jefferson University Hospital.

Dr. Folsom Elderis Board Certified in Family Medicine by the American Board of Famly Medicine.

In her spare time, she enjoys baking and reading and numerous outdoor activities, including softball and tennis and jogging and cycling. Her interests within medicine include adolescent and women's health issues, as well as newborn care.

Dr. Folsom Elder and her family reside in Prince Frederick and enjoys living and working in the Calvert County community.

Lacy Pitman, CRNP

Calvert Physician Associates is pleased to announce that family medicine practitioner Lacy Pitman, CRNP, is coming to Calvert Primary Care Prince Frederick. She will be joining Drs. J. Michael Brooks and Michelle Folsom in serving the patients of the former Calvert Family Practice Associates practice. Lacy has been with Calvert Physician Associates since 2004, most recently in the Calvert Primary Care Dunkirk and Twin Beaches practices.  Lacy will be starting in Prince Frederick on August 31, 2015.

Lacy earned her Bachelors of Science in Nursing (BSN) degree from St. Mary’s College in Notre Dame, IN and her Masters of Nursing (MN), Family Nurse Practitioner degree from the Oregon Health and Science University in Portland, OR. She began her career as a Lieutenant and Registered Nurse in the US Navy and, over her 30 plus year career has developed a passion for helping her patients live long and healthy lives.

She is a past president of the Maryland Coalition of Nurse Practitioners and is an active, elected member of Calvert Physician Associates’ governing council. She has won numerous awards in the community for her outstanding customer service in primary care and women’s health. In addition to her work with patients, Lacy is a preceptor for nurse practitioner students, serving as their mentor, teacher and instructor as they hone their skills while preparing for certification. She is a frequent speaker throughout the community, covering such topics as Women’s Health and Immunization.
